OUR SERVICES
In Home and Community Service (IHCS)
Assisting individuals with life skills in the home and community setting. Assistance with skill building, socialization, money management skills, nutrition and travel and community integration
Community Participation Support (CPS)
Providing support to individuals only in the community setting. Assistance with skill building, social skills and relationship building, money management skills, nutrition and travel and community integration
Companion Care

This service is intended to assist an individual who is age 18 or older to participate more meaningfully in home and community life. It is provided to individuals living in private homes for the purposes of providing supervision or assistance that is designed to ensure the individual’s health, safety, and welfare or to perform activities of daily living for the individual.

This service is available for individuals age 21 and older and includes comprehensive assessment, the development of and revisions to a behavioral support (BS) plan that includes strategies to support the individual based upon assessment, and the provision of interventions and training to individuals, staff, and caregivers.
​
Provides short-term relief for primary caregivers and families from caring for their loved one. This service can be provided in the home.
