About The Role
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care to individuals with physical, mental, and developmental disabilities. The ideal candidate will be responsible for administering medications, monitoring patient health, and collaborating with the care team to implement individualized care plans.
Responsibilities
Administer prescribed medications and treatments accurately and maintain comprehensive medical logs.
Monitor patients' vital signs, record medical history, and document any changes in health status. Collaborate with the interdisciplinary team to implement and evaluate individual care and behavioral plans.
Provide direct nursing care and assist individuals with daily living activities as needed.
Update patient charts and case notes daily, ensuring all documentation is accurate and timely. Educate patients and their families on health management and wellness strategies.
Develop a therapeutic rapport with individuals, fostering a safe and supportive environment.
Ensure all nursing practices comply with state regulations and protect individuals' rights.
Requirements
Active state license as a Licensed Practical Nurse (LPN).
Strong clinical assessment and documentation skills.
Excellent interpersonal and communication abilities.
Experience working with individuals with disabilities is preferred.
